Record a consultation with SOAP notes
Write a consultation with vitals and SOAP notes, set a follow-up, and mark the linked visit complete.
Updated 2026-10-02
Every consultation creates a medical record with vitals and SOAP notes (Subjective, Objective, Assessment, Plan).
Who can do this: Owner, Admin, Manager and Veterinarian ("Write SOAP notes & prescriptions"). Front desk can open records read-only.
Start a consult
- From a booking: on the clinic dashboard, click Start consult next to today's appointment. The pet, vet and reason are filled in from the booking.
- Without a booking: click New consult (
/app/clinic/records/new), search for the pet by name, and pick them from the results.
If the pet has allergies, a red banner shows them at the top.
Fill in the record
- 1Reason for visit, Date and Time (these default to now).
- 2Veterinarian: vets are listed first, then other staff.
- 3Follow-up on: set a date to put the patient on the dashboard's follow-up list.
- 4Vitals: Weight (kg), Temp (°C), Heart rate (bpm) and Resp. rate (/min).
- 5SOAP notes: S — Subjective (history and the owner's observations), O — Objective (exam findings, tests), A — Assessment (clinical interpretation), P — Plan (treatment, tests, advice).
- 6Diagnosis: a short label, for example "Acute gastroenteritis".
- 7Click Save & continue. You'll land on the record page, where you can add prescriptions and treatments.
Update and finish
- Edit any field on the record page and click Save consult.
- If the record is linked to a booking, tick Mark visit complete before saving to complete the booking. The check-out time is recorded, and the check-in time too if the pet was never checked in. If it's already done, you'll see "Visit already completed".
- Use Close case in the header when the case is finished, or Reopen case to continue it later.
Good to know
- Entering a weight also updates the pet's profile weight.
- Saving a new consult moves its booking to in progress, unless you tick Mark visit complete.
- Temperature must be between 25 and 45 °C.
- A booking can only have one consult. Clicking Start consult again opens the existing record.
- You can't close a case while the patient is still admitted. Discharge them first.
Tip: Next, write a prescription on the same page. See Write and print a prescription.
